- W2025179613 abstract "Objective: Outcome following injury to the duodenum or pancreas is frequently related to injury mechanism, time to initial diagnosis and the presence of associated injuries. Early mortality often results from exsanguination due to associated vascular injuries. It is unclear whether age influences mortality independent of mechanism or associated injuries. The purpose of this study was to determine the impact of age on mortality from injuries to the pancreas or duodenum. Methods: Retrospective review of all major trauma patients admitted to a Level 1 trauma center from July 1992 through April 2005. Age, gender, mechanism of injury, ISS, operative intervention, ICU and hospital length of stay, and mortality were assessed. Results: From July of 1992 to April of 2005, 21,200 major trauma patients were admitted to the trauma service at the University of Cincinnati. There were 52 patients (0.3%) with injuries to the pancreas and/or duodenum. Forty-one of these 52 patients (79%) were <50 years of age and 11 (21%) were 3 49 years. There were 36 males (69%) and 16 females (31%). The mean ISS was 27±2.2 and the average hospital length of stay (LOS) was 12±2.2 days. The mean ISS was lower in patients <50 years of age compared to those 349 years (24±2.4 35±4.8). Younger patients with injury to the pancreas and/or duodenum had a shorter hospital stay compared to older patients (LOS 7±2.3 days 13±2.7 days) but a longer ICU LOS (16±5.8 8±0 days). Blunt trauma was the most common mechanism in both age groups, accounting for 100% of the injuries in patients 349 years compared to 76% in those <50 years. Thirty-two operations were preformed on 52 patients (62%). Operative mortality was 12.5% in patients <50 years of age compared to 50% in patients 349 years of age. Overall mortality was 29% and was greater in the older cohort (45%) compared to younger patients (24%). Conclusions: Injuries to the pancreas and/or duodenum are uncommon. Age 349 years is associated with higher mortality and increased hospital length of stay following duodenal or pancreatic trauma. Furthermore, increased age negatively impacts survival when operative intervention is necessary." @default.
