- W2025291612 abstract "Radiofrequency (RF) energy delivery (Stretta procedure) has been shown in several studies to improve GERD symptoms and quality of life in approximately 2/3 of patients (pts). We proposed that increasing the dose of Stretta will further improve response to this therapy. Methods: Thirty six pts were randomly divided into 3 groups. Group A: Twelve pts underwent single session Stretta procedure. Group B: Twelve pts underwent sham Stretta (mirror of the active procedure in all aspects except no deployment of the electrodes). Group C: Twelve pts underwent single Stretta treatment followed by repeat Stretta if GERD HRQL was not 75% improved after 4 months. Inclusion requirements were HRQL score ≤ 11 on PPI and ≥ 20 off PPI, and 24-hour PH probe with ≥ 4% total time at PH ≤ 4. Pts with a hiatus hernia > 3 cm, grade C or D esophagitis, or esophageal motility disorder were excluded. Fifty-six RF lesions were created in each pt per session. Principal outcome was GERD HRQL improvement. Secondary outcomes were medication use, LES basal pressure, endoscopic grade of esophagitis and esophageal acid exposure. Results: See table. No serious complications occurred (no bleeding, perforation or death). However, 2 pts developed documented delayed gastric emptying after the second Stretta treatment. At one year follow up, delayed gastric emptying was improved in one pt with no improvement in the other. Summary: Mean group HRQL scores off medications and 24 hour pH scores are statistically significantly improved after active Stretta therapy after 12 months. There is a trend toward further lowering HRQL and pH scores after double dose Stretta. Sham pts only had mild lowering of their daily PPI dosages and HRQL. Conclusions: Stretta procedure is partially effective for the treatment of GERD symptoms. Double Stretta therapy has numerically superior outcome results and more durable effect compared to single therapy." @default.
