Le but de ce travail est d’étudier l’effet de la toxine botulinique injectée par voie périnéale sous contrôle électromyographique. Quatre patients tétraplégiques ont bénéficié de cette thérapie. Un examen urodynamique a été constatée, avec une diminution des phénomènes d’hyperréflectivité autonome. Aucune modification urodynamique significative n’a été relevée. À 3 mois, les résultats cliniques étaient toujours satisfaisants chez trois patients. La toxine botulinique A injectée par voie périnéale est une possibilité thérapeutique à connaître dans le traitement des dyssynergies vésicosphinctériennes. Detrusor-sphincter dyssynergia is a significant problem in patients who suffered from a spinal cord injury. It can lead to urologic complications such as hydro-ureteronephrosis or infections. Dykstra have already evaluated the ability of botulinum toxin to treat detrusor-sphincter dyssnergia with success. The toxin was injected cystoscopically. The aim of our study was to test a technique of injection through the perineum after having repaired the rhabdosphincter by EMG. Four patients who have sustained a quadriplegia were injected by 70 and 100 Units of Botox®. Urethrocystometry with electromyography was done before and after the injection. No adverse effect was reported. In three cases we noticed an improvement of bladder function. Autonomic dysreflexia decreased in three cases. We did not observe any significant changes of the urodynamic parameters. Three months later, the good clinical results remained in three patients. These results suggest that the injection of botulinum toxin throught the perineum into the urethral sphincter seems to be of great interest in the treatment of detrusor-sphincter dyssnergia." @default.
