- W2026401520 abstract "We read with interest the recent report by Papa et al, 1 Papa M. Gaspardone A. Fracasso G. Ajello S. Gioffrè G. Iamele M. et al. Usefulness of transcatheter patent foramen ovale closure in migraineurs with moderate to large right-to-left shunt and instrumental evidence of cerebrovascular damage. Am J Cardiol. 2009; 104: 434-439 Abstract Full Text Full Text PDF PubMed Scopus (36) Google Scholar who reported a significant reduction in migraine severity after patent foramen ovale (PFO) closure in patients with significant baseline migraines. The findings challenge the Migraine Intervention With STARFlex Technology (MIST) study, which showed that PFO closure resulted in no overall benefit on migraine. An aspect of the study not discussed that needs clarification is whether there was alteration in antimigraine therapy after PFO closure. We recently reported that there is a rebound increase in migraine severity early (<6 months) after PFO closure, a phenomenon more prevalent in patients with more severe baseline migraines. 2 Bhindi R. Ormerod O.J. Rebound increase in migraines following PFO closure. Catheter Cardiovasc Interv. 2008; 71: 719 Crossref PubMed Scopus (2) Google Scholar , 3 Bhindi R. Ruparelia N. Newton J. Testa L. Ormerod O. Acute worsening in migraine symptoms following PFO closure: a matter of fact?. Int J Cardiol. 2009; (Epub ahead of print) Google Scholar This early increase may reflect enhanced platelet activation before device endothelialization. It is conceivable therefore that in this study, which evaluated patients with highly symptomatic migraines at baseline, patients may have had early increases in their symptoms leading to alterations in antimigraine therapy after closure, which could confound the 12-month findings reported." @default.
