It is written for trainees and specialists in anaesthesia, intensive care and emergency medicine occasionally faced with problem paediatric cases with the aim of providing a clear plan of how to manage them. As a quick reference guide, it may also be used as a revision tool for professional examinations and as a tutorial aid for lecturers. Does the book achieve these aims? This publication, as it promises, covers a wide range of topics in paediatric anaesthesia and intensive care medicine and has good subsections on anatomy and pharmacology. Each chapter describes the fundamentals of a topic and the peri-operative considerations and management plans. The organisation is user friendly and each topic is covered thoroughly and to a high standard. There is a good case mix and the content is well presented and clearly described. The approach to each topic is to construct a case scenario with pertinent questions answered around the topic. There are also key references provided for further reading. There is a useful learning point section, which summarises key issues raised and highlights the salient details. The references are up to date and appropriate. Approximately half the chapters are appropriate for the practice of peripheral hospital anaesthetists with an interest in paediatrics and half for the specialist paediatric anaesthetists, with several pertinent to both groups. It is refreshing to see a chapter on consent and this is especially relevant nowadays and to anyone treating patients. Appropriate equipment and facilities are highlighted necessary for more complex cases. The book also describes other less obvious alternative techniques used for anaesthetizing children that may not be apparent to those less familiar with paediatric anaesthesia. Useful tables with guidelines appear consistently throughout the book, for example: details on the prophylaxis for subacute bacterial endocarditis, steroid equivalency, acute pain relief guidelines, signs and symptoms of specific illnesses in children and Glasgow coma scoring. The addition of the AVPU (alert, responsive to voice, responsive to pain and unconscious) would be helpful as this is commonly used in younger children. The book may benefit from subdividing the topics into sections, for example, a respiratory section, a cardiovascular section, and a neonatal section, etc. This additional structure would assist in the finding of relevant case scenarios and enhance its use as an aid for revision purposes. This publication is very useful for trainees in anaesthesia embarking on a paediatric module. Trainees would be well advised to read the book prior to beginning the attachment. It is also useful for the final FRCA, both as a revision aid and for planning ‘short answer questions’. As an aid for tutorials it provides a focused account of topics. Overall, this is a very useful book and an essential purchase for all trainees in paediatric anaesthesia and intensive care. It is less relevant to trainees in emergency medicine. With the trend towards in-theatre teaching, it provides a good base for discussing paediatric anaesthetic practice. Based on clinical scenarios it is interesting to read and more clinically relevant than larger textbooks." @default.
