For the last 29 years, I have been the chairman and director of an orthodontic residency program that, from its inception in 1964, has always been a 36-month certificate program (Albert Einstein Medical Center [AEMC], Philadelphia, Pa). I also had the good fortune to have been a graduate of this program. I thank its founders, Drs Maxwell S. Fogel (deceased) and Jack M. Magill, for having the wisdom and insight to understand the true benefits of a 3-year postdoctoral orthodontic educational experience. From my viewpoint, as a former resident and now as chairman and director, I wholeheartedly agree with Thomas Cangialosi. He succinctly presented a strong rationale, as did Elliott Moskowitz, as to why the time has come to move beyond discussion and proceed with formal recommendations for changing accreditation standards. Lengthening the time requirements will only enhance our ability to serve all of our various constituencies. Other dental specialty disciplines, for very good reasons, appropriately saw the need to expand the educational-clinical time frame. Being in a hospital-based academic medical center, I have always appreciated the medical model of board certification. I was gratified that the American Board of Orthodontics (ABO) moved rapidly and implemented a series of steps that now allows immediate graduates to pursue certification. My physician colleagues at AEMC have been curiously interested regarding the wide disparity in the board-certification processes between dentistry and the medical specialties, including the relatively disproportionate number of board-certified dentists compared with physicians. I agree with Drs Cangialosi and Moskowitz about the merits of a 3-year program and the opportunities presented as they relate to board certification. At our program in Philadelphia, we enthusiastically participated in the ABO Orthodontic Resident Clinical Outcomes Study (the pilot study). Of course, we at AEMC were truly pleased that all 3 of our 2004 graduates participated in and passed the oral and clinical case presentation examination, and that the next class of 3 graduates (2006) also voluntarily presented in St Louis and successfully completed this examination. If our program were less than 3 years in length, it would not have been possible for our residents to present cases from a number of choices that they fully started and completed. And to Dr Gottsegen, from my perspective and my long-time career experience that mirrors yours, you are correct in presenting such practical and critical information pertaining to contemporary orthodontics.
