Conventionally, negative pressure by a syringe (suction technique: ST) is applied to obtain more tissue, but this may hinder cytologic yield due to bloody specimen. Recently, efficacy of slow pull technique (SP) (Gastrointest Endosc.2012;75 Suppl AB183) was reported using a FNA needle with a hollowed-out reverse bevel. This slow pull technique utilizes minimal negative pressure by pulling a stylet slowly. To evaluate the efficacy of SP in a regular FNA needle, we conducted this comparative study of SP vs. ST in EUS-FNA for pancreatic malignant lesions.Patients:EUS data from April 2011, when we started to evaluate cytology specimen for each pass, to November 2012 was retrospectively reviewed. ST was a standard method for EUS-FNA, but after the introduction of SP in May 2012, a suction technique (SP or ST) as well as the size of a FNA needle were selected at the discretion of endosonographers. After EUS-FNA for both techniques, specimen was expelled onto the dish. All macroscopically visible whitish specimens were collected and put them into a formalin bottle for histological process. The remainder of the specimen was submitted for cytology. The diagnostic yield, cellularity and bloodiness of each cytological specimen were compared. Cellularity and bloodiness were routinely recorded as (−) to (3+) for each pass as a consensus between two cytologists." @default.
