Medicine is part science and part art. We have incredible scientific resources in this millennium, including diagnostic and therapeutic options that were unimaginable even a few decades ago. These resources now can be applied to help relieve the neck pain problems that confront us all. But options represent choices. Some choices are good and some are not, depending on the chooser, the chosen, and often the payer. From the standpoint of this issue of the Physical Medicine and Rehabilitation Clinics of North America, I have again been honored by Dr. George Kraft to be a chooser. A few years ago, Dr. Kraft asked me to edit an issue devoted to low back pain. After reading our book Neck Relief Now!, which was written for the lay public, he asked me to edit another issue devoted to neck pain. I knew this would be a ‘‘pain in the neck,’’ but I also felt that it was worth doing. I am very pleased with what the authors share in this issue. I have been very fortunate to be able to choose and select an outstanding panel of contributors, each of whom focuses on different segments of the issues that permit us to diagnose with reasonable confidence and to rationally treat the various manifestations of cervical diseases, derangements, and dysfunctions.
