En effet, 30 ans après avoir montré par le biais de plusieurs études, que le dépistage par radiographie thoracique et cytologie de l’expectoration n’améliorait pas la survie dans les populations dépistées, 15 ans après l’introduction du scanner volumique comme technique de dépistage dans des études observationnelles qui ont montré la sensibilité supérieure (entre 2 et 3 fois) du scanner par rapport à la radiographie pour détecter des cancers pulmonaires, les premiers résultats d’une large étude randomisée de dépistage du cancer bronchique utilisant le scanner volumique (National Lung Cancer Screening Trial) montrent que cette technique de dépistage permet de réduire la mortalité par cancer bronchique de 20,3 % et la mortalité toutes causes confondues de 7 %, en comparaison aux données du bras contrôle ayant bénéficié d’une simple radiographie thoracique de dépistage. De nombreuses questions restent en suspens concernant le dépistage de masse du cancer bronchique. Lung cancer screening has entered a new era since few months. 30 years ago, four main trials showed the lack of benefit of screening in terms of lung cancer mortality using chest radiography with or without sputum cytology. Spiral CT was introduced 15 years ago as a technique for screening lung cancer. Many observational studies showed that CT demonstrated higher sensitivity (2 to 3 times) than chest radiography for detecting lung cancer at the price of low specificity. However, the designs of these observational studies did not permit a reliable assessment of the effect of screening on lung cancer mortality. At the end of 2010, the National Cancer Institute announced that the National Lung Cancer Screening Trial (NSLT), started in 2002, was ending because the study's primary objective, i.e., benefit for the group screened by CT, was proven. In this randomized study, a 20.3% decrease in lung cancer mortality and 7% decrease in all-cause cancer mortality was demonstrated in the screened population compared to the control group undergoing chest radiography.
