This study reviews the medical records of men undergoing vasectomy at a single institution to collate data regarding PVSA results and provider recommendations for further testing. The records of all patients undergoing vasectomy by two urologists from December 2009 to August 2012 were reviewed. All patients were instructed to provide a semen sample for testing three months following their vasectomy. Demographics, post-vasectomy semen analysis (PVSA) data, and complications were recorded in a database and analyzed for descriptive statistics. The institutional financial charges for PVSA testing were also collected for cost analysis. Within the study time period, 230 patients underwent vasectomy and median patient age was 36 years old. Among the cohort, 83.5% were married at the time of their vasectomy and 95.2% had one or more children. The initial PVSA was completed by 46.1% (n=106) at a median of 15 weeks following vasectomy. Of those completing an initial PVSA, 38 patients (35.8%) had greater than zero nonmotile sperm. One patient had motile sperm seen on PVSA but was lost to further follow-up. Thirty-seven of 39 patients (94.9%) with positive results on their first PVSA could have been cleared based on the new guidelines representing a cost savings of $2300 in reduced further testing. Ten of 13 patients (76.9%) completing a second PVSA had greater than zero nonmotile sperm but all contained less than 100,000 sperm/mL. Implementation of new special clearance parameters for PVSA could prevent repeat testing in a majority of patients with positive results on initial PVSA. Prevention of further testing presents a cost savings and an opportunity to avoid the stigma of providing a semen specimen." @default.
