- W2029385718 abstract "The Free and Cued Selective Reminding Test (FCSRT) properly identifies Alzheimer disease (AD) at early stages. Different memory measures and dissimilar nature of the stimuli used for recalling have been used; however a contrast between visual (VI) and verbal (VE) versions have not been made so far. The aim of this study was to compare the discriminative validity of the free recall (FR) and the total recall (TR) for both versions of the FCSRT in a sample of patients with mild AD and controls. A convenience sample of subjects assessed with the Clinical Dementia Rating Scale (CDR) completed an entire clinical and neuropsychological evaluation. VE and VI Spanish Versions of the FCSRT, each one with different verbal or visual semantic categories, were administered separated by 7 days. Discriminative validity for the diagnosis of mild AD (CDR=0.5 or 1; MMSE≥18) for each version were estimated using a Receiver Operating Characteristic (ROC) curves analysis. 103 participants were divided into two groups: 61 AD patients (27 CDR=0.5; 25 CDR=1; 9 CDR=2) and 42 controls (CDR=0). There were no significant differences in age (74±7 versus 71.4±6; t=-1.7, p=0.07) or gender (X2=0.31p=0.86) between patients with AD and controls; nevertheless controls had a significantly greater level of education than AD patients(13±4 versus 11±5; t=2.3, p=0,02). Both versions of the test highly correlated in diverse tasks (FR: r=0.92,p<0.001, TR: r=0.86,p<0.001)with significant higher scores on VI than VE versions (VI_FR=20.1±13 versus VE_FR=15.6±12.2, t=, 8.9, p<0.001; VI_TR=37.6±12 vs VE_TR=30.4±15.2, t=9.5,p<0.001). Both versions effectively differentiated mild AD (n=45) from controls. All Memory measures had areas under the curve >0.9, without significant differences between them. By using a cut of point (COP) ≤26 the VI_FR, had a sensitivity(SE)=0.91 and a specificity(SP)=0.93. With a COP ≤22, VE_FR showed SE=0.98 and SP=0.91, while with a COP ≤45, VI_TR rated a SE=0.84 and SP=0.93and with a COP ≤38, VE_TR indicated SE=0.87 and SP=0.98. The FR for VI and VE versions of the FRSRT indicated a similar discriminative validity in differentiating mild AD from controls. The VI_TR was apparently less discriminative for identifying early AD than the other memory measures." @default.
