- W2029488181 abstract "We read the article “Neutropenia As a Potential Pharmacodynamic Marker for Docetaxel-Based Chemotherapy in Men with Metastatic Castration-Resistant Prostate Cancer” by Pond et al with interest. 1 In their retrospective analysis, the authors evaluated the prognostic value of neutropenia in 221 patients with metastatic castration-resistant prostate cancer (mCRPC) who were treated with docetaxel-based chemotherapy. The results of this retrospective analysis suggest that grade > 3 neutropenia on day 8 of cycle 1 is a prognostic marker for overall survival in docetaxel-treated patients with mCRPC. The authors suggest that neutropenia may be a pharmacodynamic marker that reflects optimal pharmacokinetics in patients with mCRPC and, therefore, they propose exploration of dose escalation of docetaxel to attain grade > 3 neutropenia. We agree with the authors that neutropenia may be an interesting marker of docetaxel efficacy in patients with mCRPC. Although docetaxel clearance is known to be associated with docetaxel-induced neutropenia, 2 the results obtained in this retrospective analysis do not necessarily show that neutropenia is a pharmacodynamic marker of overall tumor response in patients with mCRPC. It should be noted that 90% of these patients have bone metastases, in particular in the lumbar level of the spine. 3 In addition, docetaxel shows" @default.
