- W2030809904 abstract "Sir: I read with great interest the article entitled “Pacman Flap Method,” by Ritsu Aoki and Hiko Hyakusoku, which appeared in the May 2007 issue of the Journal (Plast. Reconstr. Surg. 119: 1799, 2007). As we already know, many modifications of the V-Y advancement technique have been described to achieve effective advancement of the available tissue and to decrease tension along the closure line. This method is very similar to and has the same name as the V-Y modification technique, “Pacman flap,” that my colleagues and I described for closure of pressure sores and that was published in the April 2001 issue of Annals of Plastic Surgery.1 We have usually used the Pacman flap bilaterally when large pressure sore defects were involved. Also, in our practice, we have used the Pacman flap both unilaterally and bilaterally at different sites on the body (Fig. 1).Fig. 1.: Late postoperative view of a 55-year-old patient with skin carcinoma after use of the Pacman flap.I totally agree with the authors that this modification has been used for various conditions where the defect is large or there is inadequate subcutaneous tissue. However, I believe the authors should have cited our article when considering the Pacman flap, since their article has almost the same name and the flap was used for the same purposes as those mentioned in our 2001 article in Annals of Plastic Surgery.1 Mithat Akan, M.D. Department of Plastic and Reconstructive Surgery Dr. Lütfi Kırdar Kartal Education and Research Hospital Istanbul, Turkey" @default.
