- W2031929877 abstract "DLB and AD are frequently confused in clinical practice. Amyloid pathology is required for a diagnosis of AD and is often found in DLB. Lewy bodies and dopamine neuronal degeneration define DLB but are not seen in AD. Here we evaluate the potential to differentiate Alzheimer's disease (AD) from Lewy Body dementia (DLB) by PET imaging with florbetapir F 18, an amyloid targeted tracer, and 18F-AV-133, which targets vesicular monamine type 2 transporters, a marker for presynaptic striatal dopamine terminals. On two separate days, subjects received iv injections of 370 MBq florbetapir F 18, and 185 MBq 18F-AV-133. A 10 minute PET scan was obtained 50 minutes post injection. For florbetapir, amyloid burden was estimated from cortical average to cerebellar SUVR. For 18F-AV-133 dopamine neuronal integrity was estimated from striatal to occipital cortex SUVR. 4 DLB, 3 AD and 4 healthy controls (HC) and 2 Parkinson's disease (PD) subjects have been imaged to date. Compared to HC, AD subjects showed increased florbetapir F 18 cortical SUVR but comparable 18F-AV-133 striatal SUVR, suggesting that AD subjects have selective amyloid deposition. In contrast, the DLB subjects showed both increased florbetapir F 18 cortical to cerebellar SUVR and decreased 18F-AV-133 striatal to occipital SUVR, consistent with the hypothesis that these subjects have both cerebral amyloid deposition and dopamine neuronal degeneration. The two PD subjects also showed decreased 18F-AV-133 striatal to occipital SUVR. These preliminary results suggest that combined PET imaging with an amyloid and dopamine targeted tracer can help distinguish AD from DLB patients.
