With the greater use of endoscopic procedures in the assessment of conditions of the esophagus and stomach, there has been an increase in the absolute number of iatrogenic perforations of the esophagus. Foster et al. [8] reported 1 perforation per 20,000 admissions in the period of 1950 to 1954, while the incidence rose to 1 perforation per 8,000 admissions in the period 1960 to 1964. Mark and Knauer [17] found 5 instrumental perforations in more than 700 peroral endoscopic examinations performed in a 30-month period, for an incidence of 0.7%. Wychulis, Fontana, and Payne [28] reported an incidence of 0.4% in a total of 8,038 patients undergoing gastrointestinal endoscopic procedures. The low incidence of esophageal perforation contrasts with the high morbidity and mortality that ensue when the diagnosis is delayed or when the treatment is inadequate. Youngs and Nicoloff [29] encountered an overall mortality of 26% in a series of 19 patients with esophageal perforation, but the mortality was 67% in the 6 patients in whom treatment was delayed. Other authors [13, 20, 21, 23, 271 have reported mortalities ranging from 0 to 34% in patients treated promptly, as opposed to a mortality of 68.7 to 75% in those not treated immediately. The treatment of esophageal perforation has received considerable attention in the medical literature in recent years, with the debate centering around the question of whether the so-called conservative treatment [4, 11, 19, 241 has any merit over the time-honored operative intervention [8-1 0,2 11.
