- W2034172699 abstract "OBJECTIVE: In ART, pregnancy rates are dictated, in part, by the patient profile, and Low Prognosis patients (LP) have lower pregnancy (PR) than high prognosisis (HP) patients. Literature suggests the use of D5 ET for LP (1,2,3) although Cohrane data suggest little difference between D3 and D5 ET (4). One study suggests PR increase with D2 ET in LP patients. Most literature cites clinical pregnancy and not delivery data, which is the only measure of success in ART.DESIGN: From 2005, D2 ET was used on LP (<4 oocytes, severe male or female pathology, FSH>12, BMI >38) and RF patients. High prognosis (HP) patients (not LP) had Day 3 ET. Strict sequential embryo selection (5) was used for both D2 and D3 ET and there was no difference in the mean number of embryos transferred between groups. A retrospective analysis of outcome data for delivery in LP vs. HP patients using differential ET day was conducted, with delivery as the outcome measure. The main reference point was that LP patients had <10% delivery rate prior to the initiation of the D2 vs. D3 protocol.MATERIALS AND METHODS: All non-donor or -PGD-cycles with full outcome data for 2005-2007 were included. Data was computed per transfer and delivery and loss rates calculated.Table 1Comparison of Delivery Rates for LP and HP based on Day of ETAgeLP: D2 ETHP:D3 ETP value<35 N305588+49%57%sac44%50%FHB43%48%Delivery41%46%NS35 -37 N516757+39%56.5%sac39%50%FHB37%47%Delivery35%44%P<0.0138-40 N191108+34.5%48%sac28%43.5%FHB24%38%Delivery22%35%NS>40 N8437+15%24%sac12%16%FHB8%16%Delivery7%12% Open table in a new tab CONCLUSIONS: Low prognosis and repeat failure patients can achieve delivery rates only slightly lower than high prognosis patients with the use of early transfer on Day 2, using strict sequential embryo selection, with no/few non-ET.
