Son diagnostic doit être immédiat, et sa prise en charge rapide est à la fois médicale et chirurgicale dans les formes sévères « menaçant la vie ». Nous décrivons le cas d'une interruption médicale de grossesse pour rupture prématurée des membranes à 22 SA et trois jours chez une patiente de 33 ans, deuxième geste, utérus cicatriciel ; l'expulsion se complique d'une hémorragie de la délivrance avec choc hémorragique réfractaire au traitement médicochirurgical et radiologique et pour laquelle l'utilisation du facteur VII recombinant activé (rFVIIa, NovoSeven®) a permis un sauvetage maternel. Des doses de 60 μg/kg et de 120 μg/kg ont été utilisées par voie intraveineuse en peropératoire avec un arrêt du saignement et une stabilisation de l'hémoglobine en postopératoire. Un infarctus splénique a été constaté en cours d'hospitalisation dont l'origine reste imprécise (hypovolémie, sepsis ou embolie liée au rFVIIa). L'utilisation du rFVIIa comme traitement hémostatique adjuvant de l'hémorragie de la délivrance réfractaire est très intéressante et prometteuse. Postpartum haemorrhage remains a dangerous obstetrical complication, which is the main cause of maternal mortality in developing countries. The diagnosis must be immediate and its management is both medically and surgically in life-threatening haemorrhage. We present a case of a thirty-three-year-old woman who asked a pregnancy interruption for premature rupture of membranes at 21th gestational week for her second pregnancy; she underwent a caesarean section at term for her first pregnancy. She delivered vaginally and developed a postpartum haemorrhage with hemorrhagic shock which was resistant to medical, surgical and radiological management. We decided to use recombinant activated factor VII (rFVIIa, NovoSeven®) as a final attempt to rescue the patient. During surgery, two intravenous bolus injections (60, 120 μg/kg) were successfully given with a control of bleeding and haemoglobin. The patient developed later a splenic thrombosis that can be related to either rFVIIa or to the hypovolemic shock or to the sepsis. Recombinant activated factor VII is an interesting and promising haemostatic agent in the management of life-threatening postpartum haemorrhage unresponsive to conventional treatment." @default.
