There is, therefore, great difficulty in providing for the surgeon the best possible conditions for his investigation and treatment.” This statement, made by Andre Pasquet at the Montreal Neurological Institute in 1954, reflected the difficulties associated with providing anesthesia for patients undergoing awake craniotomy for epilepsy. 24 Despite technical and pharmacologic advances, this statement remains true today. Surgical treatment of patients with epilepsy has evolved into a precise and effective therapeutic option that is no longer considered a therapy of last resort. Although these procedures are performed in an expanding number of specialized centers throughout the world, providing the conditions which simultaneously afford the surgeon a suitable operating environment while providing the patient with a safe and comfortable perioperative experience remains a significant challenge for the anesthesiologist. Seizure surgery typically involves resection of a region of cerebral cortex containing the epileptogenic focus. Several procedures are available including localized cortical resection, lobectomy, hemispherectomy, and corpus callosotomy. However, anterior temporal lobectomy is the one that is most frequently performed. Whereas some centers perform the majority of these procedures under general anesthesia, at the authors' center, cortical resection is most commonly performed under local anesthesia with intraoperative sedation. This article discusses the anesthetic considerations for patients undergoing seizure surgery while awake, focusing predominantly on patients undergoing anterior temporal lobectomy." @default.
