- W2034217894 abstract "感染性心内膜炎において,化膿性脊椎炎の合併は比較的まれであるが,致死的な合併症であることが知られている.われわれは,化膿性脊椎炎から感染性心内膜炎を診断し治療した2症例を経験したので報告する.症例1は60歳,男性.症例2は52歳,男性.ともに発熱と激しい背部～腰部痛を主訴に来院した.MRI検査で腰椎の化膿性脊椎炎と診断された.また,うっ血性心不全と弛張熱から感染性心内膜炎と診断された.2症例とも適切な抗生剤投与にもかかわらず,弁の破壊の進行および10mm以上の大きさの可動性疣贅が認められたため手術を行った.症例1は2弁置換術(大動脈弁および僧帽弁),症例2は大動脈弁置換術を行い,良好な経過を得た.2例とも術後4週間の抗生剤投与により完全にCRPが陰性化したのちに退院したが,化膿性脊椎炎に対しては,その後もさらに1～2ヵ月間の経口抗生剤投与を行った.レントゲンによる骨所見の改善とCRP正常化の維持から化膿性脊椎炎の治癒と判断し,3ヵ月後に抗生剤を中止した.その後,感染性心内膜炎,化膿性脊椎炎の再燃を認めていない.化膿性脊椎炎などの菌血症がある場合,感染性心内膜炎の合併の可能性を考慮すべきである.また,骨所見の改善するまでの長期の適切な抗生剤投与を行うことを推奨する." @default.
