Progress in the understanding of this condition, except that of therapy, has been encouraging. The association of exophthalmos and localized myxedema has been noted by many authors, * and there would seem to be a common etiology. The present concept is that this disease is not due to thyroid dysfunction but rather due to a disturbance of the pituitary-thyroid relationship. In hyperthyroidism, the thyroid-stimulating hormone of the anterior pituitary (TSH) is increased, and after thyroidectomy the TSH factor is no longer inactivated by thyroxin or some unknown factor of the thyroid.† This factor of the anterior pituitary then continues, producing exophthalmos and/or pretibial myxedema.
