- W2034451296 abstract "RATIONALE: Systemic allergic reactions to corticosteroids are relatively uncommon in comparison to their extensive use. However, immediate hypersensitivity reactions have been reported, mostly among adults, only few cases have been reported in children. We describe two children with anaphylactic reactions to methylprednisolone sodium-succinate. METHODS: Data on the clinical presentation and course were retrieved from the admission files of the children. Skin prick tests (SPT) were performed with a panel of corticosteroids preparations. RESULTS: Patient 1, 7yr old had moderate asthma and milk allergy. He developed urticaria and wheezing after accidental exposure to milk, treated with epinephrine at home and improved. Upon arrival to the emergency department he received methylprednisolone succinate IV and immediately experienced flushing and wheezing that were interpreted as rebound of his milk induced allergic reaction. The same reaction occurred again after receiving second dose of methylprednisolone succinate at the PICU. Patient 2, 12 years old with severe asthma was treated with methylprednisolone succinate IV during asthma exacerbation and experienced also flushing and worsening of his respiratory condition. Patient 1 had positive SPT to methylprednisolone sodium succinate only. CONCLUSIONS: Methylprednisolone sodium-succinate may cause anaphylactic reactions most probably due to its unique immunologic properties, possibly as a hapten, since reactions were not found to other steroids containing either the steroid itself or the salt ester - succinate. These cases emphasize the importance of awareness to methylprednisolone induced anaphylactic reactions, especially in children with asthma who may experience exacerbation or even life threatening deterioration of their primary illness." @default.
