Design: Retrospective analysis of existing data, using receiver operating characteristic (ROC) curves. Setting: Not applicable. Participants: Existing data from the AMPS International Project database, consisting of adults from numerous world regions, with varying diagnoses and levels of ability (n=64,466). Interventions: Not Applicable. Main Outcome Measure: The AMPS is a standardized assessment used to measure the quality of ADL performance. Raw motor and process ordinal scores are converted to linear ability measures through the utilization many-faceted Rasch analysis, which adjusts the final ability estimates to account for task challenge, skill item difficulty, and rater severity. Results: The ADL motor and process ability measures can be utilized to help determine level of community dependence. When using ADL motor or process ability measures to identify the need for moderate to maximal assistance, the areas under the ROC curves were 0.74 (good discrimination power) and 0.82 (excellent discrimination power), respectively. The motor cutoff measure demarcating maximum assistance was set at 1.00 logits (sensitivity=0.70, specificity=0.66). The process cutoff measure demarcating maximum assistance was set at 0.70 logits (sensitivity=0.79, specificity=0.69). The highest accuracy estimates occurred when motor and process decisions matched (n=30,835)−90% of the independent sample was correctly predicted, 22% of the sample in need of minimal assistance was correctly predicted and 88% of the sample in need of moderate/maximal assistance was correctly predicted. Conclusions: For the first time, AMPS cutoff measures demarcating independent in the community and the need for maximal assistance have been proposed.
