- W2034599210 abstract "Scabies is a common highly contagious skin parasitosis caused by Sarcoptes scabiei hominis, characterized by generalized itch. The standard diagnostic techniques for scabies consist of identification of the mite or eggs by microscopic examination of samples obtained by skin scraping with a blade and, in the past few years, of dermatoscopy. In vivo reflectance confocal microscopy (RCM) is a new advance that uses a noninvasive optical diagnostic technique to produce horizontal (en face) scans of the skin at various depths, allowing for visualization of epidermis and superficial dermis at a cellular resolution. In this 54-year-old man affected by diffuse, intense pruritus, skin scraping and dermatoscopy performed by a primary care physician resulted negative. We performed RCM that confirmed the diagnosis of scabies showing in the dorsal wrist area (Figure 1, insert), at 80 μm of depth, a female mite within a burrow (length 300 μm, width 250 μm), and enabling detailed visualization of the head (Figure 1, arrowhead), the anterior legs (Figure 1, arrows) and the peristaltic movements (see Video, Supplemental Digital Content 1, http://links.lww.com/MAJ/A62). Another RCM image capture (Figure 2) showed numerous eggs (150 × 80 μm, arrows) containing mite embryos, along with high-refractive fecal material (20 μm, arrowheads). In our case, the use of RCM enabled us to enhance the diagnosis, thus avoiding the risk of a scabies outbreak.
