- W2034687419 abstract "Objective: The aim of this study was to examine the efficacy of genetic amniocenteses (GA) performed during the 14th gestational week versus those performed during the 15th to 17th weeks by comparison of respective fetal outcomes. Methods: Perinatal ultrasound patients who underwent GA during the gestational period between 14.0 and 14.9 weeks (n = 110) were identified from a computerized registry, and their outcomes compared to patients (n = 97) of comparable maternal age and indications, undergoing GA during 15.0 through 16.9 weeks. Procedural success, multiple needle insertions, amniotic fluid leakage, early (≤30 days) and late (>30 days to term) neonatal morbidity and fetal loss were primary outcome variables. Exclusion criteria were abnormal chromosome results or major ultrasound anomalies. All procedures were performed by the same three clinicians during a 7-year period. Results: The mean biometric age for the GA group conducted during the 14-week interval was 14.6 ± 0.7 weeks, in contrast to 16.1 ± 0.7 weeks for later procedures. Contingency table analysis (χ2; or Fisher’s exact test) revealed the rate of success and number of passes to be remarkably similar in both cohorts, and not statistically significant (p = 0.531 and p = 0.392). One stillbirth occurred in the ≥15 week group. Three cases of fluid leakage were noted in the 14-week GA group (3.6 % vs. 0 %, p= 0.330). With one fetal loss as a result of PROM in the 14.0–14.9 group and none in the ≥15 group, miscarriage rates between the test groups were not significant (p= 0.885). Likewise, late complications, including PROM and preterm delivery did not statistically differ (p = 0.134 and p = 0.970). Moreover, no statistical disparities were found in procedural biometric age versus all other outcome criteria. Conclusions: Our analysis demonstrates that genetic amniocentesis at 14.0–14.9 weeks’ gestation appears to be safe and poses no greater risks than those witnessed with GA performed at later gestational ages. These findings may offer the obstetrician greater latitude when early genetic screening is indicated." @default.
