- W2034697707 abstract "ISEE-487 Introduction: While exposure to benzene has been linked to adverse health outcomes in occupational cohorts, few studies have examined the health effects of a community exposure to benzene. The Tranguch Fuel Spill has been characterized as a leakage of 50,000-900,000 gallons of gasoline from underground storage tanks in two municipalities located in northeastern Pennsylvania. As a result of the spill, it is believed that residents within an Environmental Protection Agency-defined remediation area were chronically exposed to low-level benzene from the gasoline since at least 1990. In response to community concerns, a retrospective cohort study was carried out to determine if residents were at an increased risk for cancer compared to the Pennsylvania populace and to assess the prevalence of additional health outcomes. Methods: A total of 663 individuals representing 275 households comprised the study population, constituting a 79% response rate. Demographic, health, and residential history data were collected via questionnaires. Self-reported cancers were verified by physicians and cross-referenced with the Pennsylvania Cancer Registry for the period January 1, 1990 to December 31, 2000. Age-adjusted standard incidence ratios (SIRs) were calculated using Pennsylvania rates as reference. Confidence intervals were determined using exact methods. Results: The age-adjusted all-site cancer SIR for the gasoline-exposed area was 0.82 (95% CI: 0.64-1.18) and for leukemia was 4.12 (95% CI: 1.12-10.55). Four incident cases of leukemia were reported (two acute myelogenous, one chronic myelogenous, and one chronic lymphocytic). The residential location of the acute myelogenous leukemia cases directly bordered the more concentrated areas of gasoline as projected from groundwater sampling. Discussion: These results suggest a possible association between chronic low-level benzene exposure and increased risk for leukemia among residents living near the Tranguch spill site. While acute myelogenous leukemia has been definitively linked to benzene exposure in occupational cohorts, causality in this study cannot be determined since comprehensive historical exposure estimates were not available for individual residents. As part of risk communication efforts, general precautions to lower personal environmental exposure to benzene were presented to the public in addition to the interpretation of study results in the context of identified limitations. Recommendations based on the study’s findings were made to public health agencies and the community. They included screening for disease precursors and continued surveillance for hematopoietic malignancies. Formal surveillance in the spill-affected area is essential to estimate the long-term impact of this low-level exposure to gasoline and may aid in the preservation of health within the community." @default.
