- W2034915045 abstract "In younger patients with ankle arthritis, ankle distraction arthroplasty may enable deferment of ankle arthrodesis or joint replacement. Deformity correction and distraction arthroplasty for ankle arthritis is indicated in patients younger than 55 years with posttraumatic arthritis associated with deformity of the foot, ankle, or tibia; ankle instability (without secondary bone loss in the distal tibia or talus); distal tibial nonunion; or acute injuries with a large amount of articular cartilage loss of the talar dome or tibial plafond. Retained ankle motion of at least 20 degrees should be present. Standard weight-bearing radiographs provide sufficient information for most of the ankle deformities, including tibial, ankle, hindfoot, and foot radiographs, and deformities may be quantitated with various radiographic angles including the lateral and anterior distal tibial angles. Correction of angular deformities by osteotomy involves angulating one bone segment relative to another around an axis defined as the angulation correction axis. Immediate correction of deformity is indicated for mild deformities without major shortening, soft tissue loss, or neurovascular compromise. Gradual correction of deformity is preferred for severe, complex deformities, particularly those associated with shortening, soft tissue compromise, or neurovascular risk. The preferred osteotomy technique varies with anatomic location and may consist of the multiple drill hole, Gigli saw, or focal dome osteotomy technique. After application of the ring fixator frame, ankle distraction may be done. Ankle distraction arthroplasty is successful in 70% to 80% of patients in improving pain and function. Patient selection is critical, and success is more likely with motivated, compliant patients. Patients with minimal ankle motion and marked equinus contracture are particularly susceptible to failure, and patients with previous septic ankle arthritis have suboptimal results." @default.
