- W2035024196 abstract "Background: Correct estimation of renal function is crucial in assessing prognosis of patients with HF. Recently, a new equation for estimation of glomerular filtration rate (eGFR) using combined creatinine and cystatin C levels (CKD-EPI-creatinine-cystatin C) was proposed. Objectives: To assess the prognostic role of eGFR calculated with a this equation in outpatients with heart failure (HF) over a long-term follow-up and to compare it with eGFR calculated with the Cockroft-Gault formula. Methods: The study included 879 patients: median age, 70.4 years; main etiology of HF ischemic heart disease, 52.7%; and median LVEF, 34%. Results: The formulas correlated significantly (ICC 0.877 [0.861–0.892]), but distribution of patients among National Kidney Foundation groups was different (Kappa value 0.51 [0.785; 0.604]). During a median follow-up of 3.94 years, 371 patients died. Both formulas had similar individual prognostic value according to AUC (0.668 and 0.670 respectively, p=0.72). In Cox regression multivariable analyses, both eGFR formulas remained significantly associated with mortality (HR 0.990 [0.983–0.998], p=0.010 for Cockroft-Gault and HR 0.988 [0.983–0.984], p<0.001 for CKD-EPI-creatinine-cystatin C). In the multivariable prediction model, the new formula showed better reclassification measurements in the cohort as a whole (net reclassification improvement [NRI] 4.0 [0.8; 7.1]), particularly in the subgroup with intermediate eGFR (30–60 mL/min/1.73m2; NRI 7.9 [0.7; 15.0]). Survival according to both eGFR formulas Survival according to both eGFR formulas Conclusions: The CKD-EPI-creatinine-cystatin C equation was useful for HF risk stratification and showed better measurements of performance than the Cockroft-Gault formula. This new equation may be appropriate for assessing prognosis of HF patients with moderate renal insufficiency." @default.
