- W2035075390 abstract "Benign uterine lesions are frequent causes of abnormal uterine bleeding, that need adequate diagnosis in order to choose the most conservative treatment. We set up, since October 31st 2002, an integrated “one-stop abnormal bleeding clinic” for the diagnosis of uterine pathologies. Briefly, patients are referred from the GYN outpatient clinic for abnormal uterine bleeding and undergo the same day transvaginal sonography (TVS), saline infusion sonography (SIS) and office hysteroscopy (OH). To date, 48 patients have been investigated (31 were PRE- and 17 were POSTmenopausal). SIS was impossible in 2 cases, and OH in 5 cases (stenosis of the cervix). Endometrial polyps were more frequently visualized by SIS (39.6%) and OH (37.5%) than TVS (18.7%). Myomas were more frequently diagnosed by TVS (47.9%) and SIS (49.2%) than by OH (6.25%). In case of normal TVS, SIS allowed to diagnose submucous myomas in 3 patients over the 7 cases of submucous myomas (42.9%), and endometrial polyps in 10 patients over the 19 cases of endometrial polyps (52.6%). Submucous myomas were reported in 7 patients by SIS (19.3%) and only in 1 patients by OH (3.23%). OH-guided microbiopsies and gentle curettage were performed in 38 cases. Compared to SIS, the endometrial biopsy missed the diagnosis of endometrial polyp in 8 cases over 13 (61.5%). The one-day uterine investigation concluded to the presence of DUB (16.7%), endometrial polyp (37.5%), simple endometrial hyperplasia (12.5%), myoma (20.8%) and endometrial atrophy (12.5%). DUB was treated with a LNG-IUS (25%) or endometrial resection (37.5%). Endometrial polyps were treated by operative hysteroscopy (83.3%). Leiomyomas were treated by hysteroscopic resection (70%) or hysterectomy (10%). Our original preliminary study demonstrates the usefulness of our one-day clinic and the complementarity of SIS and OH, allowing best use of conservative techniques." @default.
