- W2035161487 abstract "To evaluate the radiation attenuation properties of PC-ISO, a bio-compatible, sterilizable 3D printing material by Stratasys, and to validate a customized, PC-ISO vaginal cylinder used to treat a stage IA serous endometrial cancer patient. A custom film dosimetry apparatus was 3D-printed in PC-ISO using a Fortus 400mc (StrataSys). It was designed to hold a straight brachytherapy catheter and a 3 cm x 6 cm Gafchromic radio film segment between two blocks of PC-ISO (1 cm on each side) such that the plane of the radio film passed through the center axis of the channel. The 6 cm side of the film was radial to the channel, and the 3 cm side of the film was 2.5 mm away from the center of the channel. A brachytherapy dose plan was computed from 11 consecutive dwell positions within the apparatus, uniformly spaced 2.5 mm apart, to deliver a roughly cylindrical dose distribution to the film, which was normalized to 1500 cGy at 1 cm from the channel. The material was evaluated by comparing the film exposure to film from similar tests done in only air and only water. The planning system assumes that tissue is water-equivalent. The physician requested a custom vaginal cylinder applicator with a non-standard outer diameter of 2.75 cm. A CAD model of this cylinder was printed in PC-ISO using a Fortus 400mc (StrataSys). The average Hounsfield Unit (HU) was computed from a CT scan of the cylinder and used to verify water equivalency. The doses measured from the PC-ISO Gafchromic film test were within 1% of the dose measured in only water between 1 cm and 6 cm from the channel. The doses were increased 7-14% measured in only air over the same distances. Therefore, our tests show that PC-ISO has very close attenuation to water and that it can distinguish these results from materials that do not have the same attenuation as water. The CT scan of the cylinder had HU values between 11 and 43, which is approximately water-equivalent, and the applicator was sterilized using the Sterrad system multiple times without any damage. Given the results of the Gafchromic film test for PC-ISO and the Hounsfield Units from the CT scan, the physics and physician staff at our clinic considered the cylinder suitable for use on a GYN brachytherapy patient. We conducted a quality assurance evaluation of the PC-ISO 3D-printing material. Our evaluation showed that PC-ISO was a suitable material for a GYN vaginal cylinder in a clinical setting. Although this study was specific to PC-ISO for GYN brachytherapy, the same setup can be used to evaluate the suitability of other 3D-printing materials for brachytherapy. The customization in this study was limited to the diameter of the cylinder. Future work should explore customizing the size and shape of a cylinder specifically to the anatomy and clinical needs of a patient." @default.
