O’Connell, MD, Boston: Boston Health Care for the Homeless Program and the National Health Care for the Homeless Council, 2004, 384 pp. (paperback) The Health Care of Homeless Persons: A Manual of Communicable Disease and Common Problems in Shelters and on the Streets is both a reference tool and an eye into the world of homeless individuals in Boston. What becomes apparent when paging through this manual is that the group who has put the text together (The Boston Health Care for the Homeless Program with The National Health Care for the Homeless Council) has managed to convey a sense of community, compassion, and warmth. This second edition, edited by James J. O'Connell, MD, which has expanded from a manual on communicable diseases alone to include a broader spectrum of illness and health care concerns, is divided into multiple small chapters. These chapters are then grouped under larger headings of communicable diseases, exposure-related conditions, emerging challenges in shelter medicine, chronic diseases, immunizations, and food management in shelters. Interspersed through succinct, easily read text are colorful graphs and multiple photos, many of which have been donated by those working with the Boston Health Care for the Homeless Program. Along with a description of a disease or condition, the reader encounters photos covering a wide range of scenarios including the disease process itself, patients receiving treatment, health care providers in the outreach or clinical setting interacting with patients, and the environmental factors in Boston that may have contributed to the development of a condition. This is an engaging tactic that fosters a strong sense of community throughout the book. There are many strengths of this manual, including charts that not only list recommended treatment options for each disease, but also provide a dollar-sign rating system that estimates an expense range for each medication. Beyond a strong and practical review of infectious disease, this edition branches out into many other relevant topics including bioterrorism, appropriate use of antibiotics, nutritional issues, and treatment of chronic pain. The section on food management is a how-to public health guide on handling food donations and safe preparation of food. It is an excellent reminder that health concerns in a shelter go beyond what is seen within the clinic doors. In general, this manual refreshingly approaches health topics from an educational and preventative standpoint whenever relevant. Just like the first edition, the final section of the manual also has a collection of fact sheets in both English and Spanish, which are single-page informational sheets on the various infectious diseases discussed. The reader is encouraged to make copies of these [End Page 167] sheets and have them available for patients at the health care settings in which they work. The introduction to this manual notes that because the close quarters of shelter life often result in easy transfer of communicable diseases, the priority of this manual has been to provide a way to more readily identify early signs and symptoms of easily transmitted diseases and try to prevent further spread through education of shelter employees. If there is a weakness in this manual, it is that many areas of chronic disease are not brought up or not expanded (although it appears that there will be continued additions to chronic disease if there are further editions.) For example, mental illness is represented solely by a chapter on depression, but there is no discussion of pharmacologic treatment of depression. Diabetes is addressed, but hypertension is not. Although the editor of this book obviously has a challenging task in choosing which topics to include, it seems as though the most prominent sections missing are possible chapters on both substance abuse and domestic violence or trauma-related sequelae because (as mentioned at multiple points throughout the manual) these are two highly comorbid conditions that frequently influence care of other diseases encountered.
