- W2035842029 abstract "Abstract The handling of urea in the renal concentrating operation is compared with that of a nonpermeant solute (mannitol) over a physiologic range of solute excretion. Comparisons are made in the hydropenic state during vasopressin administration in twelve subjects on an 80 gm. protein ward diet and in three subjects whose protein intake is varied. The rate of solute-free water reabsorption (T c H 2 O) was approximately the same for any given osmotic clearance (C osm ) with urea and mannitol diuresis over 2 ml. per minute despite wide variations in the concentration of urea in the urine (U ur ) from less than 50 to over 300 mM per L. Enhancement of concentrating capacity (defined as an increase in T c H 2 O at a given C osm or the osmolality of the urine at flow rates below 1 ml. per minute) by exogenous urea in subjects on a low protein diet was dependent on the flow rate of urine. It was observed only at flow rates below 2 ml. per minute in association with a high reabsorption fraction of filtered urea (from 55 to 65 per cent). No enhancement of T c H 2 O occurred at higher rates of flow during prolonged urea diuresis, even when U ur increased from low control levels to high diuretic levels. It is suggested that during osmotic diuresis in man the behavior of urea in the concentrating operation approaches that of a nonpermeant solute and that under these circumstances the urinary concentration of urea exerts little direct influence on the medullary urea concentration." @default.
