- W2036466114 abstract "I read with great interest the article by Famuyide et al [ 1 Famuyide A.O. Breitkopf D.M. Hopkins M.R. Laughlin-Tommaso S.K. Asymptomatic thickened endometrium in postmenopausal women: malignancy risk. J Minim Invasive Gynecol. 2014; 21: 782-786 Abstract Full Text Full Text PDF PubMed Scopus (16) Google Scholar ], which addressed a debated topic—the management of asymptomatic postmenopausal women with a thickened endometrium. They studied 155 women with an endometrial thickness >4 mm with no vaginal bleeding undergoing diagnostic hysteroscopy and showed a rate of endometrial cancer or atypia of 1.29% (only 2 cases, both on 2 endometrial polyps with an endometrial thickness ≥17 mm). Based on these results, the authors concluded that “our data support the need for the evaluation of the uterus for structural or focal lesions, and when endometrial polyps or focal lesions are diagnosed, they should be removed even in the absence of a history of vaginal bleeding” [ 1 Famuyide A.O. Asymptomatic Thickened Endometrium in Postmenopausal Women: Malignancy RiskJournal of Minimally Invasive GynecologyVol. 21Issue 5PreviewTo assess the diagnostic findings and determine the frequency of malignancy in postmenopausal women evaluated by office hysteroscopy for a thickened endometrium without bleeding. Full-Text PDF ReplyJournal of Minimally Invasive GynecologyVol. 22Issue 4PreviewWe appreciate Dr. Giannella bringing attention to their article, which was published simultaneously with ours. We believe that there is further work needed in this field, and, certainly, a well-defined endometrial thickness cutoff for identifying cancer among asymptomatic women would reduce the need and costs of evaluation. We did not have the statistical power (with only 2 cases of atypia or cancer) to define that cutoff. Given our findings and those of Giannella et al [1], an 8-mm cutoff would be appropriate to rule out malignant and premalignant conditions. Full-Text PDF" @default.
