- W2036581608 abstract "Prostate biopsies performed with transrectal ultrasound guidance (TRUS) are based on the recognition that approximately 70% of cancers are located in the peripheral zone, and prostate ultrasonography allows for definition of this area. Though initially six biopsies were obtained of the peripheral zone in patients with an elevated prostate specific antigen (PSA) or abnormal digital rectal examination (DRE), experience indicates that the greater the number of biopsies the greater the likelihood of diagnosing malignancy. Today, most physicians obtain eight to ten cores in patients undergoing their first biopsy, and in those requiring repeat biopsy, a greater number is usually obtained not only of the peripheral zone but also of the transition zone. In those patients with palpable nodules or in those with hypoechoic areas in the peripheral zone, additional biopsies are obtained of these regions. These techniques have resulted in increased diagnosis in patients with elevated PSA and negative DRE. Radiation therapy traditionally is administered by external beam, and prior experience with brachytherapy was unsuccessful likely due to poor seed placement. With the development of transrectal ultrasonography as a means of guiding seed placement, there has been a rapid increase in the utilization of brachytherapy in treating patients with localized disease. Oftentimes performed alone or in combination with external beam, success rates have been greatly improved and data indicate comparable tumor-free intervals to the surgical experience in up to 10 years of follow-up." @default.
