- W2036686073 abstract "Opportunistic infections (OIs) are the hallmark of the immunodeficiency associated with HIV infection. In the absence of effective antiretroviral therapy, the main strategy for the management of AIDS was the prevention of opportunistic infections. Successful strategies were developed to prevent Pneumocystis carinii pneumonia (PCP), disseminated Mycobacterium avium complex (MAC) infection, and cerebral toxoplasmosis; prophylaxis for these conditions was recommended as the standard of care for susceptible patients. In addition, chemoprophylaxis was shown to be effective in other situations (e.g., fungal infections, CMV disease), although cost–benefit analysis and other considerations led to recommendations that prophylactic therapy not be used routinely, but reserved for selected patient populations. The widespread use of preventive therapy led to a decline in the incidence of several OIs even before the current era of effective treatment for HIV. The use of potent antiretroviral therapy, however, has resulted in a more dramatic change in the epidemiology of OIs. A significant and sustained decline in the incidence of OIs has occurred, with a concomitant drop in HIV-associated mortality.59 This article reviews prophylaxis of OIs in the era of potent antiretroviral therapy." @default.
