We assessed the accuracy of preoperative radiological prediction of preserved renal mass using a novel CT image-rendering program (3D Synapse - Fuji film©). Correlation is made with the volume of the resected specimen and post-operative renal function. METHODS We identified 100 patients undergoing laparoscopic partial nephrectomy (LPN) for a solitary renal mass. The preoperative CT scan was assessed using advanced image-processing software to measure the volume of both kidneys, the tumor volume and to calculate the predicted volume of preserved renal mass by subtracting the predicted resection volume. The predicted resection volume included the tumor as well as a surgical margin of normal renal parenchyma (see image). The actual resected volume was calculated by measuring the volume of the specimen on the back table following excision. Predicted postoperative eGFR was calculated by multiplying the preoperative eGFR with the predicted functional remaining volume (FRV). Predicted postop eGFR = Preop eGFR × % FRV RESULTS All patients successfully underwent zero-ischemia LPN, with or without robotic assistance. Mean age was 60.5 years. The mean tumor volume was 21.3 ml, mean predicted resected volume (tumor volume + predicted resection margin) was 45.7 ml and mean predicted FRV was 92.5%. Comparing the predicted resected volume with the actual resected volume demonstrated excellent correlation, with a Spearman Correlation Coefficient of r=0.91 (p<0.0001). The mean preop eGFR was 80.4 ml/min. The mean predicted postop eGFR was calculated using the above formula and was found to be 74.6 ml/min. There was excellent correlation between predicted postop eGFR and the actual postop eGFR, r=0.93 (p<0.0001). Reproducibility between surgeons was excellent with an interobserver correlation of r=0.98 (p<0.0001) for predicted resection volumes. CONCLUSIONS We describe a novel technique of pre-operatively predicting the surgically resected volume and the predicted post-operative eGFR. Good correlation with the actual resected specimen and the actual post-operative eGFR is seen. Advanced image-processing software demonstrates excellent applicability for pre-operative surgical planning for complex procedures such as PN.
