- W2038032583 abstract "Aim: The aim of this study was to report a series of patients with Stress cardiomyopathy triggered by different medical or surgical therapeutic interventions. Background: Stress cardiomyopathy is a recently described entity characterized by reversible left ventricular dysfunction in response to acute stress. Exaggerated sympathetic stimulation is believed to play a central role in the pathogenesis of this condition. Methods: Patients diagnosed with this condition were evaluated and followed prospectively between January 2006 to January 2009. All patients underwent coronary angiography and noninvasive cardiac investigation including transthoracic echocardiogram. Possible triggers or stressors were investigated. Results: We observed 29 consecutive cases diagnosed with Stress cardiomyopathy. In almost half of the individuals (14 patients, 48.27%), a medical therapeutic intervention (pseudoephedrine or beta-2 adrenergic agonist treatment) or a surgical procedure (liver, renal biopsies, minor surgeries) was identified as the potential stressor. Patients were mainly female, (27 females, 2 males) mean age of 68 years SD 7.96 and all but two had T wave inversion in the ECG.The totality of the patients showed troponin I elevation (mean peak TnI 1.68, SD 1.77), decreased ejection fraction (mean EF 33.92%, SD 11.97) and no obstructive coronary artery disease in the coronary angiogram. Conclusions: Stress cardiomyopathy commonly presents as a complication of medical and surgical interventions. Increased catecholamine secretion in response to stress generated by therapeutic interventions may play a role in the pathogenesis of this entity in these cases. Prevention, early recognition and avoidance of certain medications are imperative in the management of these patients." @default.
