En la técnica habitual, el ayudante quirúrgico o el propio cirujano han de sujetar el testículo con la mano no dominante durante toda la intervención. Con el dispositivo en forma de torniquete de Rumel descrito por nosotros para estabilizar el contenido escrotal durante la operación, los testículos quedan fijos y apretados contra la pared escrotal en todo momento, sin tener que sujetarlos con la mano durante la intervención. El dispositivo escrotal se construye con un drenaje de goma tipo Penrose y un trozo de tubo de plástico. Se coloca en la base del escroto alrededor de ambos testículos y se ajusta la tensión hasta conseguir que la piel esté tensa y la pared escrotal, bien ajustada contra los testículos. Generalmente no hace falta colocar el separador palpebral, ya que las diferentes capas de la pared escrotal se separan a medida que se incide. Hemos utilizado el dispositivo en 32 pacientes a los que se practicó una biopsia testicular, y en todos ellos el dispositivo facilitó el acto quirúrgico, que se pudo realizar sin ayudante quirúrgico. En 19 pacientes la biopsia se realizó para obtener espermatozoides para ICSI por azoospermia obstructiva; en 3 casos, por azoospermia secretora y en los 10 restantes, para examen histopatológico para completar el estudio de infertilidad. No hubo ninguna complicación atribuible al dispositivo. Este sencillo dispositivo escrotal simplifica el procedimiento, ya que estabiliza efectivamente los testículos durante toda la operación, no se requiere ayudante quirúrgico, el cirujano puede emplear ambas manos en la intervención y permite obtener la biopsia testicular a través de una incisión muy pequeña. The main current indication of open testicular biopsy is the extraction of sperm cells for ICSI in azoospermic patients. Usually, either the surgical assistant or the surgeon himself has to hold the testicle with his non-dominant hand throughout the operation. We propose using a scrotal device in the shape of a Rumel tourniquet in order to maintain the testicle fixed and tight against the scrotal wall all the time, with no need to be held by the hand. The scrotal device is made up of a Penrose-type rubber drain and a piece of plastic tube. It is placed at the base of the scrotum so as to include both testicles while tension is adjusted until the skin becomes tense and the scrotal wall is held tightly against the testicles. Placing the eyelid retractor is unnecessary, since the various scrotal wall layers become spontaneously separated. We have used this device in 32 consecutive patients. In 19 patients testicular biopsy was done in obstructive azoospermia to retrieve spermatozoa for ICSI and in 3 cases for histopathological diagnosis, in the remaining 10 patients biopsy was done to complete the histopathological study for infertility, the Rumel tourniquet was very useful in all of them. No device-related complications were observed.
