- W2038815314 abstract "A291 Cytokines play a central role in directing both the magnitude and type of immune response generated to renal transplants. Since they are normally expressed at low or undetectable levels but rapidly up regulated at the onset of immune response, they should provide early predictors of graft dysfunction. Early acute rejection is an important complication of renal transplantation. In this retrospective study we investigated whether the recipient serum levels of cytokines (sIL-2, sIL-2 R and sTNF alpha) are useful predictor of renal graft outcome. Serum levels of cytokines were determined pre- and post-transplant in 57 renal transplant recipients and correlated with clinical findings. Patients were separated based on the HLA-DR mismatches. Graft survival, acute rejection and serum creatinine at 12 years were analyzed for comparison between groups. Cytokines were measured by ELISA (Roche). High post-transplant sIL-2R was found in 63% of cadaveric renal recipients. The incidence of acute rejection was 77% in high IL-2R group compared to group without rejection incidence 23% (p=0.0056). In subset with high sTNF-alpha (23%) 84 % patients have episode of acute rejection (p=0.038). sIL-2 cytokine did not present significant correlation with acute rejection. Among one HLA-DR mismatched patients (51%), sIL-2R and TNF-alpha were more strongly associated with acute rejection. Graft survival was similar between zero and one HLA-DR mismatch. In this study we have shown that patients with high level of sIL-2R and TNF-alpha in post-transplant serum may have an influence on acute rejection of renal transplants." @default.
