A detailed evaluation of the intracranial anatomy of the surviving twin was performed by multiplanar transfontanellar sonography preferably with a transvaginbal probe. Follow-up was obtained in all cases. Ten monochorionic twin pregnancies with single fetal demise ranging in gestational age between 22 and 28 weeks were seen, most frequently in the context of twin transfusion In one case the surviving twin died in utero. In 2 cases, delivery was expedited soon after the demise. In one case the position of the surviving twin was unfavourable for analysis. In 4/6 remaining cases abnormal cerebral sonographic findings were identified including one case each of subdural hematoma with ventriculomegaly, periventricular diffuse echogenicities, grade IV hemorrhage, brain atrophy. Two pregnancies were terminated and the remaining fetuses were delivered and survived. Prenatal diagnosis of brain lesions was always confirmed and all affected infants have severe neurologic sequelae. Two fetuses had normal cerebral structures, which was confirmed postnatally by both sonography and MR and are following normal developmental milestones at 1 and 5 year. Of the three cases that did not undergo antenatal neurosonography, two developed porencephaly and are now affected by severe neurodevelopmental delay and cerebral palsy. Overall, brain injuries were present in 7/9 survivors. Albeit limited, our results confirm a very high rate of brain injuries in fetuses surviving after the intrauterine death of a monochorionic co-twin. Fetal neurosonography is a valuable tool for the prediction of the neurologic outcome in such cases." @default.
