- W2040049943 abstract "We compared the cumulative incidence of metachronous lesion after endoscopic submucosal dissection (ESD) of gastric adenoma or early gastric cancer (EGC) in the single lesion group and the synchronous multiple lesions group at ESD. A total of 561 patients with gastric adenoma or EGC who had undergone ESD treatment were periodically followed up with endoscopic examinations for at least 1 month. The patients treated surgically were excluded from the study. In the case of resection margin(+), lymphatic invasion(+), vascular invasion(+) or perineural invasion(+), The patients were excluded from this study. The metachronous lesion was defined as new gastric adenoma or EGC which occurred in the different location from the initial lesion. The mean follow-up period of endoscopy was 13.7±13.0 months in the single lesion group (n=524) and 10.8±9.4 months in the multiple lesions group (n=37). The first metachronous lesion occurred in 32 patients (6.1%) after a mean of 54 months in the single lesion group, and in 6 patients (16.2%) after a mean of 28 months in the multiple lesions group. The overall culmulative incidence of the first metachronous lesion in the single lesion group was significantly lower than that of multiple lesion group (P=0.004). The cumulative incidence of the first metachronous lesion within 6 months from ESD in the single lesion group was significantly lower than that in the multiple lesions group (3.1% vs 10.8%, P=0.036). Also, the cumulative 1-year incidence in the single lesion group was significantly lower than that in the multiple lesions group (4.6% vs 13.5%, P=0.018). In order to detect metachronous gastric adenoma or EGC, careful endoscopic follow up within the first year after ESD is necessary, particularly in the synchronous multiple lesions group at ESD. Especially, short term follow up (within 6months) is more important to detect metachronous lesions." @default.
