- W2040103448 abstract "Evaluation of fetal cerebral cortex sulcation is important for the prenatal diagnosis of disorders of neuronal migration (e.g. lissencephaly). An abnormal Sylvian fissure is frequently observed in these conditions. Currently, prenatal diagnosis of an abnormal Sylvian fissure is subjective. The objective of this study was to develop an objective parameter to evaluate the fetal Sylvian fissure. This cross-sectional study included 78 singleton fetuses with no chromosomal or anatomical abnormalities. Using multi-planar, 3D ultrasound, the Sylvian fissure midpoint was identified concurrently in the axial and coronal planes. Measurements were performed in the transverse plane from the midpoint to the inner surface of the parietal bone, perpendicular to the falx cerebri. Bland–Altman plots were used to determine intra- and inter-observer agreement between two independent observers. Regression analysis was used to evaluate the correlation between the measurements and gestational age. Seventy-seven of 78 (98.7%) pregnancies had a clearly visible Sylvian fissure, which was identifiable from 12 weeks of gestation. The mean Sylvian fissure distance at 12 and 40 weeks of gestation was 1.4 mm and 13.4 mm, respectively. Intra- and inter-observer mean differences between paired measurements were 0.01 mm (95% limits of agreement −0.41 to 0.43 mm) and 0.05 mm (95% limits of agreement −1.79 to 1.90 mm), respectively. The Sylvian fissure parameter and gestational age fit a linear regression model with a multiple R = 0.85, and a R2 = 0.72 (Sylvian fissure distance to inner surface of the parietal bone = −2.465 + 0.433*gestational age). 1) The distance from the Sylvian fissure to the inner aspect of the parietal bone can be reproducibly measured from 12 weeks of gestation to term; 2) this parameter increases with gestational age; 3) the Sylvian fissure to parietal bone distance may be of value in the prenatal diagnosis of neuronal migration disorders." @default.
