- W2040608035 abstract "Endometrial carcinoma in young women using oral contraceptives is rare. In this case study the patient was a 33-year-old para 2 woman with a 3-month history of menorrhagia and intermenstrual bleeding. Altogether she had taken oral contraceptives continuously for 7 years i.e. Norlestrin (norethisterone acetate plus ethinyl estradiol) for 18 months then Serial 28 (ethinyl estradiol and megestrol acetate) and finally Serial C. Tissue obtained by curettage showed a highly differentiated adenocarcinoma. Hysterectomy and bilateral salpingo-oophorectomy were performed. Myometrial invasion had not taken place. Radium therapy was used postoperatively. 4 years later she remained well. The major action of all sex steroids at a cellular level is the induction of protein synthesis. To initiate this action the steroid must bind to a specific polypeptide receptor in the cytoplasm. Tissue progesterone receptors are normally induced by estrogens. Absent or deficient progesterone receptors in an area of the endometrium would leave that area under the sole influence of estrogen. This has been shown to be more common with sequential preparations. Although high-dose progestagen therapy has been reported to be effective in anovulatory infertile women with highly differentiated endometrial carcinoma and carcinoma-in-situ has disappeared after induction of ovulation and pregnancy surgery seems to be the only rational form of treatment." @default.
