- W2040731472 abstract "OBJECTIVE: To compare pregnancy rates (PR) and live birth rates (LBR) between patients undergoing transfer of thawed pronuclear (PN) embryos (frozen for OHSS prevention) and patients undergoing fresh embryo transfer (ET). DESIGN: Retrospective. Academic practice. Study period 1/97 to 12/07. MATERIALS AND METHODS: Data from 3621 IVF cycles were reviewed. Fifty-six patients were at risk for OHSS based on estradiol (E2) > 3500 pg/ml on day of hCG administration and/or symptoms. Eighteen were cancelled, and 38 cryopreserved embryos. Thirty-five, included in this analysis, have undergone ET. The control group consists of 284 patients with normal ovarian reserve who had a fresh ET in 2006 at MGH. Groups were compared with regard to age, diagnoses, day 3 follicle stimulating hormone (FSH) and E2, insemination type, amount of medication, day of hCG, peak E2, number of oocytes, PR and LBR per retrieval. RESULTS: Groups were comparable with regard to diagnosis, D3 labs, day of hCG, and insemination type. The cyrothaw group was younger, received less medication, had a higher peak E2, and had more oocytes retrieved than controls. Controls had one ET, whereas 16 cryothaw patients had 2 or more. Comparison of PR and LBR between groups shows that there is a higher PR (77 vs 51%) and LBR (51 vs 38%) among patients who had cryopreservation. Given that several factors affect IVF outcome, particularly age, multiple logistic regression was performed adjusting for age, diagnosis and IVF type. Cryothaw patients had significantly higher PR than controls (odds ratio = 3.1, 95% CI 1.3 to 7.1, p = 0.008), had PR on the first transfer that was not statistically different than controls (odds ratio = 1.7, 95% CI 0.8 to 3.7, p = 0.15), and had LBR that was not statistically different than control patients (odds ratio = 1.0, 95% CI 0.5 to 2.1, p = 0.97).Table 1FreshCryothawp-valueAge (years)35.2±3.733.6±3.30.0209Day 3 FSH6.9±1.96.3±2.10.1677Day 3 E241.4±16.844.1±17.00.4943ICSI %50%46%0.7219 (Fisher exact)Medication36.2±15.324.0±12.00.0001Day of hCG11.7±1.512.1±2.00.3044Peak E221966.8±675.33957.8±968.0<0.0001Oocytes10.8±5.118.8±7.1<0.0001Pregnancy146 (51%)27 (77%)see textLive birth rate108 (38%)18 (51%)see textMean +/- standard deviation, p=t-test unless specified Open table in a new tab Mean +/- standard deviation, p=t-test unless specified CONCLUSIONS: Cryopreservation, for patients at risk for OHSS, offers a safe alternative to fresh ET with excellent probability of live birth." @default.
