Its bacterial origin was described in 1882 (1) and the first anti-bacterial agents were identified in 1932 (2) leading to the current wide choice of such agents. It is perhaps then surprising that review articles still need to be written about the subject. Unfortunately, despite advances in our knowledge we are still unable to answer fundamental questions about the pathogenesis of the condition, the nature of the interaction between the causative organisms and the host, and why patients continue to die. This latter point emphasizes the need for early and appropriate treatment in all patients with pneumonia. In industrialized countries most deaths occur in the elderly and in the presence of other chronic diseases, but it is often forgotten that young adults also die, with 1318 deaths recorded between ages 15 and 64 in England and Wales in 1989 (3). Many deaths occur early in the course of the illness with a quarter of those who eventually die doing so within the first 48 h of hospital admission (Fig. 1). The aims of management are therefore primarily to prevent death but also to minimize morbidity and to hasten recovery. This article describes the factors to be considered in the management of pneumonia. However, in a condition with multifactorial aetiology occurring in a wide variety of patient groups it is difficult to provide absolutely precise guidelines." @default.
