It affects how well dermatologists discharge this obligation and, at a more personal level, what dermatologists do, how they do it, where they do it, and how much they get paid for doing it. The issues are multiple, their solutions far from clear and, for these reasons, controversial. This article presents one view of these issues, gathered by the author as Chair of the Workforce Study Group of the American Academy of Dermatology (AAD) Program for Dermatology for the 21st Century and through discussions with colleagues throughout the United States. The problems and their solutions discussed in this article are personal opinions of the author and do not reflect the official position of the Committee or of the AAD. Dermatologists are not powerless in dealing with these issues.
