- W2040842236 abstract "Recent studies suggest that central artery augmentation index (AI), a composite measure of the amplitude and timing of the arterial reflected pressure wave, is positively associated with serum C-reactive protein (CRP) and inversely associated with exercise capacity in healthy adults. In heart transplant recipients (HTRs) during the first year after transplantation, elevated CRP is an independent predictor of cardiac vasculopathy and mortality, and exercise capacity remains 60-70% of age-matched healthy subjects. PURPOSE: To determine whether central artery wave refection properties are associated with CRP and exercise capacity in HTRs early after transplantation. METHODS: Two months after heart transplantation, 20 HTRs [age=54.4±2.3 yrs; sex=15M/5F; body mass index, BMI=26.2±0.7 kg/m2] reported to the laboratory after an 8 hour fast. Aortic pressure waveforms were synthesized using a validated generalized transfer function obtained from high-fidelity non-invasive pressure waveforms of the radial artery via applanation tonometry. Central AI was calculated as the ratio of aortic augmented pressure to aortic pulse pressure and was corrected for heart rate. The roundtrip travel time of the reflected wave to and from peripheral reflecting sites (ATp) was calculated as the time duration between the diastolic foot and the infection point of the aortic pressure wave. Fasting venous blood samples were obtained to determine serum lipids and plasma inflammatory proteins and a symptom-limited treadmill test with oxygen uptake (VO2) was performed to determine exercise capacity. RESULTS: AI and δTp were positively (r=0.68, p=0.001) and inversely (r=−0.62, p=0.003) associated with logCRP, respectively, but not with interleukin-6, tumor-necrosis factor-α, HDL, LDL, total cholesterol, age, blood pressure, or BMI. AI was also inversely associated (r=−0.54, p=0.016; r=−0.51, p=0.027) and δTp was positively associated (r=0.47, p=0.04; r=0.57, p=0.01) with peak VO2 and exercise test duration, respectively. Furthermore, CRP was inversely associated with exercise test duration (r=−0.65, p=0.003) and peak VO2 (r=−0.43, p=0.06). CONCLUSIONS: Increased arterial pressure wave refection is associated with elevated CRP and reduced exercise capacity in HTRs during the early post-transplantation period." @default.
