- W2041133990 abstract "Neoglottic reconstruction began soon after the first laryngectomy was performed in 1866. A satisfactory resolution of the problem eludes surgeons to this day. Many surgeons prefer to delay neoglottic reconstruction after laryngectomy for the following reasons: (1) the surgeon at the primary operation devotes full attention to removing the cancer; (2) pharyngeal mucosal closure may be compromised by the additional incisions and sutures required to perform elaborate neoglottic reconstructions, risking pharyngocutaneous fistulas; (3) early attempts at phonation, with increased pharyngeal pressures, increase the strain on the pharyngoesophageal suture line; (4) patients are denied the opportunity to learn esophageal speech, the most satisfactory form of phonation, after laryngectomy; and (5) when esophageal speech cannot be learned, tracheoesophageal puncture can be precisely placed without being distorted by the scarring that sometimes accompanies healing." @default.
