- W2041348564 abstract "Introduction Diffuse malignant mesothelioma arises from the serous cavities, the most common site being the pleura. Although most researchers agree that there is a background incidence of malignant mesothelioma, the most significant aetiological agent is asbestos. The early diagnosis of mesothelioma is therefore important not only for medical but also for medicolegal reasons. Diffuse mesotheliomas are always malignant. They display a wide range of growth patterns, cellular composition and degree of cellular differentiation. They are classified by the World Health Organization as epithelial, mesenchymal or fibrous, and mixed. The various appearances of diffuse mesotheliomas have been well described (1). The epithelial form is predominant in most studies (2). Mixed tumours show both epithelial and mesenchymal features. The proportions between the two subtypes are usually unequal and the distribution is not uniform. The diagnosis of malignant mesothelioma is known to be difficult. Both adenocarcinomas of the pleura (metastases and peripheral lung cancers) and mesothelial hyperplasia often present considerable problems to the pathologist. Although it is still not uncommon for diagnosis to be made at autopsy (and not before), thoractomy and open biopsy are generally accepted as means of making a definitive diagnosis. Thoracoscopy is an attractive alternative to open surgery because of its low complication rate and the possibility to obtain material from several selected areas, permitting staging. Accuracy rates close to 100% have been reported (3,4). The value of closed needle biopsy in the diagnosis is questioned. A success rate of 60% was claimed by Edge and Choudhury (5), but lower figures are usually reported (l&S).
