Though uncommon, the incidence increases with advancing age, becoming the most frequent testis cancer in men over 50. We sought to characterize these patients and tumors in the Veterans Affairs Central Cancer Registry (VACCR). METHODS The VACCR is a repository for cancer data from 120 participating VA medical centers. We analyzed data from 1995 to 2008 for all testicular tumors (ICD-O-3 sites C62.X) and identified all testicular lymphomas. For each patient, the VACCR includes 130 fields including demographic information, tumor histology, treatment, and vital status. Descriptive analysis of patient data was performed. RESULTS We identified 155 patients with testicular lymphoma from a database of 1283 patients with testicular tumors. The median age at diagnosis was 70 years (range 43 to 91). By race, the population was 94% white and 4% black. 24% were active smokers and 24% actively use alcohol. Documented exposures include agent orange (3%), ionizing radiation (1%), and asbestos (2%). Histologic subclassification was available in 144 (92%) of cases. 110 (85%) were diffuse large B-cell lymphomas, with other histologic subtypes much more rare, including 4 (3%) small B-cell lymphocytic, 2 (2%) Hodgkin lymphoma, and 1 (1%) Burkitt lymphoma. 130 patients (84%) had staging information, with stage I (49%), II (13%), III (8%), and IV (29%). 12 (8%) of the lymphomas involved both testes. Extranodal involvement was seen in 50 (31%) of these patients, with 6 (4%) presenting with CNS involvement. Orchiectomy was performed on 140 patients (90%), with 35 patients (23%) receiving additional radiation therapy; chemotherapy was administered to 109 patients (70%). The overall survival at last followup was 36%. The overall survival by stage was I (50%), II (35%), III (45%), IV (24%). Mean follow-up was two years, with median followup of 10 months. 69 patients (45%) were never rendered disease-free, while 51 patients (33%) have no recurrence in the followup period. 14 patients (9%) had recurrence of their disease, most commonly to distant extranodal sites. CONCLUSIONS Testicular lymphoma is a rare neoplasm that presents primarily in older men. The VACCR is a large and contemporary dataset that includes a large number of patients with PTL.
