La contamination de l'ascite, habituellement par des germes entériques, avant tout Escherichia coli, est soit directe, soit indirecte par voie hématogène. L'hypertension portale et l'insuffisance hépatique créent des conditions hémodynamiques et immunologiques qui expliquent la survenue de ces infections. La responsabilité adjuvante des actes invasifs et des hémorragies digestives est probable. Le tableau clinique est rarement très suggestif, et c'est l'examen au moindre doute du liquide d'ascite qui est le geste fondamental. La définition des critères d'infection reste dans certains cas difficile, le chiffre de polynucléaires neutrophiles (supérieur à 500 mm3) paraissant les meilleurs marqueurs, même en cas de culture négative (30 % des ascites infectées). La détermination de groupe à risque d'infection est une voie prometteuse, permettant d'envisager une prophylaxie en particulier chez le malade ayant un taux dans l'ascite bas de protides fibronectine et C3. Le traitement antibiotique par les Céphalosporines de 3ème génération paraît adéquat mais même en cas de succès infectieux, l'évolution peut être défavorable du fait de l'insuffisance hépato-cellulaire dont la sévérité reste un élément pronostique fondamental. The infection of the ascites fluid (or spontaneous bacterial peritonitis) is a serious progressive complication of decompensated cirrhosis. The contamination of the ascites usually by Enterobacteriaceae is done above all by Escherichia coli. This contamination can either be direct or indirect by the hematogenous way. Portal hypertension and hepatic insufficiency create hemodynamic and immunological conditions which explain the coming of these infections. The adjuvant responsability of invasive acts and digestive hemorrhages is probable. The clinical picture is seldom very suggestive and it is the examination, at the slightest doubt of the ascites fluid which is the fondamental act. The definition of criterious for infection remains difficult in certain cases : the polymorphonuclear leukocyte figure (above 500/mm3) appears to be the best marker even in the event of negative cultures (30% of infected ascites). Determinating a group at risk for infection is a promising track which enables to consider a prophylaxis, in particular with the ill person having in the ascites a low lend of fibronectine and C3 proteins. The antibiotic treatment with third generation Cephalosporins appears to be appropriate but even in the event of a success against the infection, the evolution may be unfavorable as a result of the hepato-cellular insufficiency for which the severity remains a fondamental element of prognosis." @default.
